Gege answered that it is normal for Jujutsu sorcerers to get scars as part of their job. This might mean that Utahime got her scar in the long run of her career as a Jujutsu sorcerer. She might have got it as part of a mission.
What Might have Resulted in Utahime’s Scar?
Looking back into the way of Jujutsu Kaisen, we find that Utahime didn’t have her scar in the Hidden Inventory arc. This is evident when we see Utahime and Mei Mei in the arc as they get trapped inside the haunted mansion. Even after Gojo, Geto, and Shoko are sent to save them and the mission is over, she isn’t harmed.
Considering that the Hidden Inventory arc took place in 2006, and we meet Utahime later in the first season of Jujutsu Kaisen during the Kyoto Goodwill Event arc, which happened around 2018. This would mean that Utahime got her scar between 2006 and 2016, probably as part of a mission.
The chances for Utahime getting a scar in 2016 are minimal, as the scar didn’t look fresh during the arc. This would mean that she must have received it at an earlier stage. Taking Gege Akutami’s statement into consideration, Utahime must have got the scar during a mission or fight.

Even as students, we see Gojo calling Utahime weak and annoying her. Even in the Kyoto Goodwill Event arc and later, we see the same kind of exchanges. We also see them working together to find the mole inside the school.
Utahime is also a powerful character as she has the ability to enhance her cursed energy as well as the others around her. The effectiveness can be raised to almost 120 percent. She is also able to use a curtain, a barrier capable of hiding the things within from the outside world.
