Things took a darker turn with this week’s episode of The Patient. Despite several therapeutic tactics suggested by Alan, nothing seems to curb Sam’s urge to kill.
In the end, he kills Elias, and Alan, in shock, can only fear that sooner or later, Sam will find him guilty of not being able to cure him and end his life too.
1. Episode 6 Synopsis/Speculation
Dr. Strauss struggles with the reality he finds himself in; Sam is consumed with practical necessities but agrees to his therapist’s advice to make different choices.
With Episode 5’s ending, Alan is only more convinced that soon Sam may find him guilty of not being able to cure him, but his role is not at an end in Sam’s life yet. Episode 6 may have scheduled a little breakdown of sorts for the doctor after witnessing a murder firsthand.
Alan’s new angle on the similarities between Sam and his abusive father may have worked if the hasty plan hadn’t been sprung by the therapist.
Alan may be distraught, but his years of experience may yet prove significant in turning Sam’s thoughts around using that connection. So there’s more of that surely in the upcoming episode.

5. Episode 5 Recap
Sam visits Mary, his ex-wife, upon Dr.Strauss’ behest and thus ensues an awkward conversation. Elias proposes Alan and himself take Sam by surprise and escape.
Alan leaves a message for his daughter and son with Elias, his fate too clear to him. In a vulnerable moment, we see more of Alan’s backstory with his son, daughter, and late wife. Sam returns from Mary’s and talks with Alan, letting him know that while it was nice to visit his past, it didn’t accomplish what Alan hoped it would.
Alan calls for Candace as Sam proceeds to kill Elias once and for all. Alan notices a new dynamic between the mother and son, mentally noting it.
Alan finds a different approach to explain Sam’s “violent behavior” and asks him to “search more deeply” to pinpoint the root of his anger. Alan connects Sam’s way of thinking to his father by explaining their violent behavior with a vague reason.
Alan asks Sam to have Elias join their therapy session, and Sam obliges. Alan tries to show another side of Elias to Sam, but to the therapist’s terror and dismay, Sam kills Elias in front of him.

Therapist Dr. Strauss is kidnapped and imprisoned by his patient Sam who needs him to find a therapeutical solution to help him stop his homicidal tendencies.
