Itadori and Mahito fight it out in episode 21 of ‘Jujutsu Kaisen’ season 2 titled “Metamorphosis.” Mahito deliberately splits up Itadori and Todo again but fails to keep them apart.
In his awakened state, Mahito releases his innate domain, ‘Self-Embodiment of Perfection.’ Todo is out of the battle but not without dealing some severe damage to Mahito alongside Itadori.
However, Mahito transforms further, making it hard for Itadori to take him down. As the battle progresses, so does Itadori’s drive to kill Mahito. He successfully lands a fatal hit, and the roles between them reverse.
Itadori is now a predator in a world where Mahito is the prey, and Mahito does what he does best – run away.

2. Episode 22 Speculation
With Mahito now reduced to nothing, Itadori plans on killing him. However, Geto’s sudden appearance surprises both Itadori and Mahito. Geto was fighting MeiMei, but considering he is now here, MeiMei probably lost. We might see the recap of the fight between them.
Since Geto offered to save Mahito, he might have a plan in mind. However, from everything we know about the original Geto, he cannot use reversed curse energy. However, since a cursed spirit has possessed Geto, it might be safe to assume that the cursed spirit can use it.
Since Geto suffered from severe injuries in his fight with Yuta and was as good as dead, the cursed spirit might have used the Reversed Cursed Technique to heal his body. We will see how he plans on saving Mahito. Possessed-Geto could also make Mahito a part of his curses.
3. Recap of episode 21
Mahito splits himself again, one half engaging Todo and Itadori in battle while the other uses Soul Multiplicity. He creates a transfigured human and manages to separate Todo and Itadori again. Todo defeats one of them but is attacked by two more.
Meanwhile, Itadori fights Mahito alone, who plans to take Itadori down while Todo is absent. However, Todo returns, much to Mahito’s annoyance. Mahito uses an innate domain expansion for 0.2 seconds to avoid touching Sukuna and getting killed.
Todo uses a simple domain to protect himself, and Itadori rushes forward to exorcise Mahito before he can use Idle Transfiguration. However, instead of using his technique after activating the barrier, Mahito merges these two steps and targets Todo’s hand.
Todo reacts fast and cuts off his hand. Mahito speeds towards him and attacks Todo with a Black Flash. Todo concentrates his entire cursed energy on his stomach to protect himself. However, this time was enough for Mahito’s cursed technique to recover, so he charges at Todo again.
Todo’s locket breaks open, revealing Takada and Itadori’s picture, which surprises Mahito. Todo takes this chance to beat Mahito up. He slaps his palm against Mahito’s, switching places with Itadori, who hits Mahito with a full-force Black Flash. However, touching Mahito directly affects Todo’s palm.
Itadori immediately takes off after Mahito, who finally realizes the true shape of his soul and has an awakening. Mahito transforms into a more complex and grotesque form than any of his previous transformations, naming it the “Instant Spirit Body of Distorted Killing.”
Itadori and Mahito charge towards each other, but Mahito’s body is stronger than before. Itadori’s attacks do not have the same effect as before, and he realizes Mahito is no longer just a cursed spirit.
Both Mahito and Itadori took serious damage from the previous fights, but that will not stop them. Itadori plans on using Black Flash even though there has been no sorcerer who could use a Black Flash at will. However, the current Itadori might be capable of doing so.
Mahito reverts half of his body to throw off Itadori, but Itadori attacks with a time-lag Black Flash. Before Mahito can counter, Todo returns and baits Mahito into thinking he can still use Boogie Woogie. This gives Itadori enough time to use a final Black Flash, completely overpowering Mahito.
Mahito reduces to a husk of himself, out of cursed energy and transfigured humans. Itadori admits that he and Mahito are the same. Itadori wanted to reject Mahito’s words because he did not want to kill without reason or meaning.
However, Itadori no longer wants any of it, as he has accepted his role as just a cog in a wheel much larger than his existence. Itadori’s words have reduced Mahito into a pathetic state of fear as he runs away yet again. Now, Itadori is the predator, and Mahito is the prey, the hunter, and the hunted.
Mahito makes a last desperate attempt at saving himself by throwing dirt and rocks at Itadori, who stands over him, unfazed at Mahito’s screaming and crying. As Mahito crawls away, he lands at Geto’s feet, who asks if he should save Mahito. Itadori stops in surprise at Geto’s appearance.
