Boruto confronts Kiseru head-on in episode 280 of Boruto, titled “Breakthrough”.
As the buildup finally concludes, Boruto lays out all the facts to Kiseru’s face and fights him head-on. They move to a secret underground tunnel and fight in the darkness as Kiseru outpowers them one by one.
Plot twists after plot twists make this episode a confusing feast for the eyes as we find that nothing we’ve watched so far has been confirmed. Only time will tell what even is the truth, and hopefully, Boruto will find out soon.

Episode 281 Speculation
As Boruto finally breaks out of the genjutsu, he will likely be able to face the true mastermind of the entire game. Now that Sarada, Mitsuki, and Kawaki are also on the same page, he will probably have support in his battle.
The others will likely remain inside the genjutsu until Boruto’s fight is done, as there is no way for them to get out from the inside now that Kiseru is incapacitated.

Recap of Episode 280
Boruto confronts Kiseru about his alliance with Ouga, stating all the facts and saying it straight to his face. However, Kiseru decides to run away instead of countering his argument.
The rest of the team admires Boruto’s diligence, and Namua seems unwell, and he talks to Yatsume about his final wishes, and the gate begins closing. But Namua goes and holds it open, calling it his atonement.
Everyone crawls out and goes to help Boruto, as Namua remains behind. Kiseru struggles to escape as they surround him and confront him without any escape. He decides to rely on ninjutsu and runs into an underground tunnel.
In the darkness, he launches multiple surprise attacks against the group, fights Rokuro one-on-one, and his fire style outpowers him completely. As he goes to finish him off, Boruto comes in clutch with his Rasengan.
Anbu ambushes Sarada, Mitsuki, and Kawaki on the way; Kiseru takes out Yatsume with ease as Shamo watches in fear. However, she does not go down easy as he uses lightning jutsu in the water, and Shamo quickly follows up with his Earth style.
Boruto is entrusted to stop him. The Anbu come in peace as they exchange intel with the group. Kiseru is revealed to be a member of the Anbu as a spy, and he is sent in to find information.
As countless Jonin have gone missing in the past, Shamo also discovers that Kiseru isn’t evil. As Boruto faces off against Kiseru, he starts analyzing the situation and deuces the phrase, “Don’t trust anything.”
He is slowly losing his mind as he begins fighting against Boruto, both seemingly evenly matched as Boruto outplays him with his shadow clone jutsu. Shamo interrupts but gets caught in the crossfire.
Kiseru deduces that the whole maze is a genjutsu as he stabs Boruto. This initiates the lock of the actual lair, and Boruto walks out of his chamber, seeing multiple more of the same with everyone trapped within a mass genjutsu.
